Outreach
What is
Outreach? Social workers provide
information and referrals to services and programs for individuals and
families in accordance
with prescribed programs of the Senior Services Department.
Outreach
programs and services may be explained by
telephone, by a pre-arranged office conference or during a visit to
your home.
Outreach workers can provide information and assistance with many programs
including community resources, care-giving issues, flu shots, health fairs, nutrition counseling
information, community loan closet equipment, File of Life, legal assistance,
HEAP (Home Energy Assistance Program), Bethlehem Food Pantry, WRAP (Weatherization
Referral Assistance Program), assistance with Food Stamp certification,
assistance with Telephone Lifeline, SSI (Supplemental Security Income)
information, EPIC (Elderly Pharmaceutical Insurance Coverage), Medicare Part D
and Medicare Premium Savings plans (QMB and SLMB)
